Cost of Corporate Flu Program

	Regular           $ 30/employee by  9/30
	Early Bird     $ 20/employee by  7/30

Some other discounts or charges may apply. After 9/30 rates may vary up to $40/employee.

Due to recent significant  increases in the manufacture's price of the flu vaccine serum,  Corp-med will bill the cost of the vaccine separately. Corp-Med assures our clients that we will do our best to obtain  a competitive volume discount for serum. 

On-Site fees  $100/hour
Additional fees: Physician supervision of Flu vaccination: $200/hour. 

On-Line Scheduling for corporate flu program: $3/employee (cost includes set-up, scheduling and confirmations). 

E-Mail Services: There is a $100 set-up fee for standard mail broadcasts.

Reserve

	Early Bird special: Full payment is required by July 30th.
	Schedules will be confirmed after payment is received. Regular Flu vaccinations: 75% pre-payment is required at reservation. A minimum charge of $350 for on-site vaccinations. Off-site vaccination programs are also available at regular rate for less then 50 employees.10% discount for more then 1000,  additional discount may apply for nationwide programs).
	In case of supply shortage certain additional charges may apply, these charges are passed on from suppliers and providers. Corp-Med bills clients cost of vaccine with no markup.
	* Guaranty and Refunds  Our fees are based on pre-booking.  In order to offer volume fees, the number of vaccinations booked need to be guarantied by our clients. Therefore we cannot give refunds.  Additional  participants will be billed at regular  rate. Orders can be rescheduled but are non-cancelable or refundable.
